Services

  • Medicaid Waiver

    The ACS Waiver is a Medicaid funded program for individuals with developmental disabilities with an age of onset before age 22. Rather than living in an institution like the Conway Human Development Center, the ACS Waiver allows individuals with developmental disabilities to live as independently as possible in the community. The program is designed to support and assist individuals in acquiring, retaining and improving the skills and abilities necessary to live independently in home and community-based settings.
  • Therapy

    Speech Therapy

    Speech therapy is provided by a full-time Speech Therapist. The Speech Therapist provides evaluation of communication skills, direct intervention for articulation/language deficits, and development of alternative/augmentative communication systems in order for the individual to effectively communicate with others at work, home and in the community.



    Occupational Therapy

    Occupational Therapy is provided by a full time Occupational Therapist who provides assessment and treatment of fine motor skills, activities of daily living and sensory integration. Intervention focuses on improving the individual’s ability to function as independently as possible.



    Physical Therapy

    Physical Therapy is provided by a part time Physical Therapists who provides assessment and treatment of gross motor skills including ambulation, range of motion, balance, and muscle tone. Recommendations may be made for assistive devices such as wheelchairs, walkers and orthotics.

  • Case Management

    Planning for Success

    The case manager establishes a meeting to develop a plan of care and service plan. This meeting includes the case manager, the person to be served, his or her parent or guardian, and the direct support staff. This meeting ensures that everyone involved understands the service planning process and their active role in the process.

    Based on the individual’s desires, an Individual Service Plan (ISP) is developed. The plan contains a long range (3yr – 5yr) personal goal as well as a set of annual goals. Plans are then made to remove or overcome any barriers that may interfere with the individual achieving his or her personal goals.

    Focusing on Strengths & Desired Outcomes

    A process will be followed to identify the services, resources, needs and wants of the individual receiving services so the resulting plan will be based on the person’s strengths, needs, preferences, and desired outcomes.

    Our team of case managers initiate and oversee the process of assessment and reassessment of the individual’s needs and desires and the review of plans of care.

    Achieving Goals

    A data collection system is developed to indicate an individual’s progress as it relates to goals. Our case managers oversee services provided and closely monitor collected data to determine the progress toard achieving identified goals and objectives. Changes may be made to the individual’s plan of care at least once a year to ensure progress towards achieving goals.

    Short term objectives are developed based on the individual’s long range and annual goals. These objectives describe the steps and expected outcomes needed to reach the annual goals. Objectives contain initiation dates, target dates, end dates, criteria for success, methods and materials to be used, and the person(s) responsible for the implementation of each objective...

  • Transportation

    With no public transportation in Faulkner County, our consumers depend on our fleet of over 40 vehicles.
  • Residential Services

    Residential Services are provided to allow people with developmental and intellectual disabilities to live in the most independent setting consistent with their abilities.
  • Health and Wellness

    We offer several programs to promote health and wellness.
  • Day Services

    Profiles Enrichment Center

    Profiles Enrichment Center, our adult development program, provides adults with developmental/intellectual disabilities the opportunity to further their learning either in a classroom environment or in a pre-vocational setting.

    Profiles Productions

    Profiles Productions offers positions in recycling as well as contract work from various companies in the central Arkansas area.

  • Supported Employment

    Community Access Supported Employment is an innovative approach to assisting individuals with developmental and intellectual disabilities in obtaining gainful employment in Faulkner County. Our program provides five core resources to support new employees and employers:

    • Joint Opportunities Building Success (JOBS+)

      Preparatory classes provide a basic set of skills to acclimate the candidate to working in the community.

    • Job Development / Job Coordination

      Job developers assist the consumer in finding a job that caters to their personal interests and abilities as well as catering to the employer’s needs. Consumers receive assistance in finding a job, applying for a job and interviewing for a job.

    • Job Coaching

      Job coaches create a supportive training atmosphere to help the consumer learn the job. They work hand-in-hand with the employer to ensure the consumer is trained properly and understands all job tasks.

    • Transportation

      Transportation is provided to and from the job site for program participants.

    • Extended Services

      Consumers are supported throughout the life of their job to ensure long term success.
